This was started in November 2023 (see previous minutes)

The team consists of a Doctor/GP, Nurse, Trainee and Care Navigator. Their roles are to prioritise appointments/telephone calls and onward referrals based on clinical decisions.

This helps with case management and allows for a rapid response. The number of phone calls to practice has massively reduced which, in turn improves efficiency – see attached report.  There is still some fine tuning to be done, but the Practice is very happy to receive any constructive criticism in order to support patient journeys.

The Practice is trying to get continuity and support for patients with chronic conditions using AI, but this needs to be done alongside the ICB (Integrated Care Board).

Friends & Family texts don’t seem to be getting through to all patients. It would be helpful to have some more positive feedbacks to be on the internet.

From the beginning of February 2024 GP Practices in England will be working in conjunction with local Pharmacies to see and treat seven conditions to include:

·         Uncomplicated UTI

·         Shingles

·         Impetigo

·         Infected insect bites

·         Acute sore throat

·         Acute sinusitis

·         Acute Otitis media

The Pharmacists can manage appointments once the Practice has referred the patient and will only charge normal prescription fees if you currently pay them. All exemptions are taken into account.

Direct deliveries/Online Pharmacies are damaging local pharmacies and this is a way of supporting them. Local pharmacies are very keen to be involved and so far this has been working well.

Note:

The normal repeat prescription system still applies with GPs authorising prescriptions for chronic/ongoing conditions for a year – patients will need to discuss this with the Practice by contacting them directly.
